This form should not be used for urgent problems, it is to help with routine information gathering for your baby's 6-8 week check and we cannot guarantee when your answers will be reviewed by a doctor. If you have any urgent concerns or need to talk with a doctor before your appointment, please contact the surgery.
In advance of your 8 week baby check, here is some information we require to help ensure your appointment runs smoothly and to allow us to focus on the baby’s needs during the current pandemic whilst restricting the amount of time within the practice to reduce the risk of potential exposure to the coronavirus for baby, parent/carer and our staff.
We appreciate you may not have all the information, but please answer as many questions as you are able to.
